Community Paramedicine (CP) is a community-based health care that evolves based on each community's individual needs.
The Johnston County Community Paramedic Program is an innovative way to care for patients outside the hospital.
This Program is comprised of paramedics with 80 hours of specialized training in addition to their paramedic training. This expanded level of training allows the Community Paramedic to collaborate more effectively with all members of the patient's healthcare team in order to support the needs of each patient.

Live in Johnston County
Referred into the program by a case manager and have a diagnosis of Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, or Diabetes.
Utilized EMS 4 or more times in a 30 day period.
Utilized the emergency department 7 or more times in 6 months.
