What is a Community Paramedic Program?

Community Paramedicine (CP) is a community-based health care that evolves based on each community's individual needs.

The Johnston County Community Paramedic Program is an innovative way to care for patients outside the hospital.

This Program is comprised of paramedics with 80 hours of specialized training in addition to their paramedic training. This expanded level of training allows the Community Paramedic to collaborate more effectively with all members of the patient's healthcare team in order to support the needs of each patient.

Similar programs in the United States have produced remarkable results.

Does this cost me money?

We are funded by the Duke Endowment Grant. Therefore, our program is at no cost to you.

Do you come to my home?

Yes! We make appointments at your convenience

Can you come on the weekends?

Yes! We are available seven days a week, from 7am to 7pm.

Do you come to my house on an ambulance?

NO. We will come to your house on a Ford Explorer clearly marked as a Johnston County Community Paramedic.

Who qualifies for the program?

  • Live in Johnston County

  • Referred into the program by a case manager and have a diagnosis of Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, or Diabetes.

  • Utilized EMS 4 or more times in a 30 day period.

  • Utilized the emergency department 7 or more times in 6 months.

  • Help with medications

  • Understanding a new diagnosis

  • Avoid recurring hospital visits

  • Avoid recurring EMS utilization

  • Avoid recurring ED utilizations

  • Facilitate communication with your healthcare team

  • Connect you with available resources
